Partnership to provide a technology and cultural model of care strengthen PathfinderHealth’s primary care practices
FLAGSTAFF, Ariz., November 29, 2022 /PRNewswire/ — PathfinderHealth, a subsidiary of Northern Arizona Healthcare (NAH), today announced a partnership with Equality Health, a leading provider of value-based physician networks uniquely equipped to serve the needs of diverse and historically underserved populations. Effective January 1, 2023the partnership will provide enhanced patient and caregiver services to PathfinderHealth’s primary care practices in the North Arizonaand assist PathfinderHealth, a clinically integrated network and accountable healthcare organization, in its transformation to value-based healthcare initiatives.
After an extensive national search, Equality Health was selected to partner with PathfinderHealth to consistently increase healthcare provider satisfaction and improve patient care and health outcomes. Equality Health will also provide PathfinderHealth with new resources to manage total healthcare costs through a localized, whole-person, cultural model of care. Equality Health’s unique cultural care model and the companies’ shared mission to provide quality care to underserved communities forged the relationship.
